1. Copy of Passport or Green Card

2. Both address since 15 years old

3. Both employment information since 15 years old

4. Copy of Vietnam Passport of the person you plan to marry, the number, place of issue, date of issue of ID card number of the person you plan to marry. (Information used to fill out the application)

5. If divorce, need divorce certificate and place of divorce certificate, need to certify or original, reasons for divorce 
6. Social Security Number
7. Both father's place of birth (Quê quán)
8. Both parents' and siblings' name, date of birth, ethnic group,  nationality, place of origin, and address.
9. Years to be searched to order a marriage letter of no record.
10. 2 photo pictures (2x2) for both
